                      One-Day Root Cause Analysis Workshop

We are pleased to offer the Charles Brooks Associates, Inc. one-day Analytical Thinking
and Root Cause Analysis workshop for professionals who want to learn specific methods of
identifying problem causes and producing solutions.  Your investment for the complete, in-
house workshop will be $2000.00, plus out of pocket expenses for our workshop presenter.  
Since the workshop is a very intense, interactive process, we ask that you limit the
attendance of each workshop session to a total of not more than twelve participants.

The Charles Brooks Associates, Inc. one-day Analytical Thinking & Root Cause Analysis
workshop classroom presentation includes the following elements – as a minimum:

                                              Agenda

              I.        Problem Definition

              II.        Developing Possible Causes
                         - Fishbone, Cause and Effect                                                       

              III.        Data Collection and Organization

              IV.        Analysis
                          - Data Review
                          - Five Why's

              V.        Solution      

In addition to the classroom session, which will consume approximately six hours, the
workshop attendees will be asked to work together to select a common manufacturing
problem, perform the brainstorming required, choose the best alternative, and produce a
solution to the problem.  Techniques applied will include use of the Fishbone, Cause and
Effect Diagram; Data Analysis; and 5 Why analysis.  

Prior to the workshop, Charles Brooks Associates, Inc. will send you a questionnaire
requesting that the workshop participants state their specific goals and objectives for the
session and asking them to outline a problem or problems that they would like considered
for solution by the  
workshop participant team.  We are always available for individual or small group guidance
and coaching after the group session, if that should be desired.

You should plan to furnish a meeting room with computer slide presentation capability; a flip
chart; and an erasable white board.  We suggest that you and your staff furnish
refreshments for breaks and lunch as a part of the day’s activities.  Workshop attendees
can receive a complimentary copy of the workshop slides and other reference materials by e-
mail after the conclusion of the session.
